The RFI Questionnaire is the spreadsheet that goes with the Request for Information document. It gathers, as scored questions, what the buyer asks the market before sending an RFP or RFQ. Each answer scores YES = 1, PARTIAL = 0.5 or NO = 0, multiplied by the weight of the question; the Summary tab says whether the vendor is pre-qualified and the Comparison tab ranks several vendors side by side.

How to Use This RFI Questionnaire Template (PMBOK 8)

1. Fill in the RFI data in the RFI Details tab.
2. Adjust the categories in the Param tab and write the questions, with category and weight, in the Questions tab.
3. Send the spreadsheet to the vendor. The vendor fills in their data and the Answer column.
4. The Summary tab calculates the score and tells whether the vendor is pre-qualified.
5. In the Comparison tab, paste each vendor's result to see everyone's rank.
