6. In-progress Postmortems

7. After-action Reviews

After-action reviews (AARs) are structured reflection sessions conducted after a significant event, activity, or phase to capture lessons learned while the experience is still fresh. Originally developed by the U.S. military and later adopted in project management, AARs follow a disciplined four-question framework that distinguishes them from general debriefs or retrospectives: What was planned? What actually happened? Why was there a difference? What can we learn and apply going forward?

In PMBOK 8, after-action reviews are recognized as a lessons learned and continuous improvement tool applicable across all project approaches — predictive, adaptive, and hybrid. Unlike retrospective meetings (which are sprint-cadenced) or in-progress postmortems (which are incident-triggered), AARs are completion-triggered: they are conducted after a defined unit of work concludes. The four AAR questions

  1. What was planned (intended outcome)? A precise statement of the plan, goal, or expectation that was in place before the activity. This anchors the review in facts rather than memory and prevents post-hoc rationalization.
  2. What actually happened (observed outcome)? A factual description of the result — what was actually delivered, how it compared to the plan, and what unplanned events occurred.
  3. Why was there a difference (root cause analysis)? The analytical core of the AAR. The team examines both positive deviations (why did we perform better than expected?) and negative deviations (what caused the gap?). Root cause analysis tools — 5 Whys, fishbone/Ishikawa diagrams, timeline analysis — are applied here.
  4. What will we do differently (lessons for the future)? Specific, actionable recommendations that will be documented in the lessons learned register and applied to future work. Vague observations (“communicate better”) are rejected in favor of specific practice changes (“add a daily 10-minute sync between the infrastructure team and the feature team during integration sprints”).

AARs vs. retrospectives: the key distinction

Both AARs and retrospectives are structured reflection tools, but they differ in focus and methodology. Retrospective meetings are team-process focused — they examine how the team worked together and what practices should change. AARs are mission/outcome focused — they examine what happened during a specific activity and why, using a rigorous cause-effect analytical framework. 8. Review Meetings

Types of review meetings in PMBOK 8 contexts

9. Daily Coordination Meetings

Daily coordination meetings are brief, focused synchronization events held every working day to align team members on progress, plans, and impediments. Information radiators vs. dashboards

What should radiate

Not all project information belongs on a radiator. Effective information radiators display:

  • Impediment board (current blockers and who owns resolution)

Sensitive information (individual performance data, budget details, personnel matters) does not belong on a radiator visible to visitors or stakeholders outside the core team.

11. Visual Controls

Visual controls are any technique that makes the current state, progress, or condition of work visible at a glance, enabling immediate detection of normal vs. abnormal conditions without reading text or numbers. The concept originates in Lean manufacturing (the Japanese term “andon” — a signal light that turns red when a process deviates from standard) and is integrated into PMBOK 8 as both a quality and team coordination tool.

Visual controls are distinct from information radiators in their primary purpose: information radiators communicate project state broadly; visual controls specifically signal when action is needed. A kanban board is an information radiator. The work-in-progress (WIP) limit on that kanban board is a visual control — when a column exceeds its WIP limit, the visual control signals that the workflow has deviated from the standard and intervention is required.

  • Definition of Done checklists: Physical or digital checklists visible at each team member’s workstation that must be completed before a story can move to “Done” are visual controls — they prevent incomplete work from advancing by making the quality standard visible at the moment of decision.

Why visual controls matter for project performance

Visual controls reduce the cognitive load required to assess project health. Rather than reading a detailed status report to determine whether the project is on track, a well-designed visual control system allows a 15-second scan of the team workspace to reveal the current state. This immediacy enables faster response to problems: deviations from plan are visible within hours rather than discovered at the next status meeting. The faster a deviation is detected, the cheaper it is to correct.

12. Velocity

Velocity is the measure of how much work a team completes in a single iteration, expressed in story points. It is calculated by summing the story points of all user stories accepted as “done” (meeting the Definition of Done) at the end of a sprint. Velocity is one of the most important metrics in PMBOK 8’s adaptive measurement toolkit — it is both a planning input (how many story points can we commit to in the next sprint?) and a forecasting tool (when will the remaining backlog be complete?)

A team’s velocity is team-specific and context-specific. It cannot be meaningfully compared between teams or used to assess individual developer productivity. A velocity of 40 points for Team A vs. 80 points for Team B tells us nothing about their relative capability — the two teams almost certainly calibrate their story point estimates differently. Velocity is valuable as a longitudinal metric for the same team over time, not as a cross-team benchmark.

Story points and the Fibonacci sequence

Story points represent relative effort and complexity, not hours. The Fibonacci sequence (1, 2, 3, 5, 8, 13, 21) is the standard scale for story point estimation because its non-linear gaps reflect a real property of complex estimation: the difference between a 1-point and 2-point story is meaningful and small, but the difference between a 13-point and 21-point story is enormous and increasingly uncertain. The non-linear scale encourages teams to decompose large stories (anything above 8 or 13 points should be split) rather than treating them as planning-ready.

Using velocity for forecasting

Velocity-based forecasting is straightforward: divide the total remaining backlog points by the team’s average velocity to estimate the number of sprints required. If the backlog contains 320 points and the team’s 6-sprint average velocity is 38 points, the forecast is approximately 8.4 sprints — about 17 weeks at two-week sprints. This forecast is probabilistic, not deterministic: teams typically use a range (based on minimum and maximum recent velocities) rather than a single number, producing a confidence interval for delivery.

PMBOK 8 explicitly recognizes this velocity-based forecasting as the adaptive equivalent of earned value schedule performance management, providing a data-driven basis for delivery commitments and scope negotiation with sponsors and stakeholders.

Velocity anti-patterns

Using velocity as a performance metric to compare team output over time creates perverse incentives: teams inflate story point estimates to show higher velocity without actually delivering more value. This is “velocity gaming” and it destroys the metric’s usefulness as a planning tool. PMBOK 8’s guidance on velocity is clear: it is a planning and forecasting tool, not a performance target. Teams should aim for stable, predictable velocity rather than maximum velocity.

13. User Stories

User stories are concise, user-centered descriptions of a desired software capability written from the perspective of the person who will use it. They are the primary unit of work in agile and iterative projects, representing the smallest independently deliverable unit of customer value. In PMBOK 8, user stories are recognized as a key output of the Manage Product Backlog process and as the fundamental building block of adaptive scope management.

The standard user story format is:

As a [type of user], I want [some goal or action], so that [some benefit or reason].

For example: “As a registered customer, I want to save my payment method, so that I can complete future purchases without re-entering my card details.” The three-part structure is not merely syntactic convention — each part serves a deliberate analytical purpose: the user persona anchors the story in a real need, the goal describes what the user wants to accomplish (not how the system should implement it), and the benefit articulates why this matters, creating a basis for prioritization and for evaluating whether proposed implementations actually satisfy the need.

Acceptance criteria

Every user story must have acceptance criteria — specific, testable conditions that must be satisfied for the story to be accepted as “done.” Acceptance criteria translate the vague “I want” into observable, measurable outcomes. For the payment method story above, acceptance criteria might include: (1) the system displays a “Save payment method” checkbox during checkout; (2) when checked, the card is tokenized and stored against the user account; (3) returning customers see saved cards as a selection option at checkout; (4) saved cards can be deleted from the user’s account settings; (5) the system complies with PCI DSS storage standards.

Acceptance criteria serve multiple purposes: they complete the story definition (resolving the ambiguity left by the high-level “I want” statement), they provide the QA specification for test design, they define the conditions the product owner uses to accept or reject the story at sprint review, and they communicate explicitly what is out of scope for this particular story.

The INVEST criteria for user story quality

INVEST is the quality framework used to evaluate whether a user story is ready for sprint planning:

  • Independent: The story can be developed and delivered without depending on another story being in progress or completed. Dependencies between stories create scheduling complexity and reduce the team’s flexibility in sprint composition.
  • Negotiable: The story is not a fixed requirement. The “I want” describes the desired outcome, not the implementation. The team and product owner negotiate the optimal way to satisfy the need within the available effort.
  • Valuable: The story delivers clear value to the identified user type. Stories without articulable user value are scope padding — they should be challenged or removed.
  • Estimable: The team can estimate the story’s size in story points. Stories that cannot be estimated have too many unknowns and require technical investigation (a spike) before estimation is possible.
  • Small: The story is small enough to be completed within a single sprint. Stories larger than half a sprint’s capacity should be decomposed using story splitting techniques.
  • Testable: The story has concrete acceptance criteria that can be verified by QA. Stories without testable acceptance criteria are not ready for development — they will be accepted or rejected on subjective judgment.

Epics and features

User stories exist within a three-level backlog hierarchy. Epics are large, high-level user needs that are too vague or too large to implement directly — they are progressively decomposed into features (specific product capabilities) and then into user stories (sprint-ready implementation units). “As a user, I want to manage my subscription” is an epic. “As a subscriber, I want to upgrade my plan, so that I can access premium features immediately” is a feature. The individual stories that implement the upgrade flow (select new plan, enter payment, confirm upgrade, receive confirmation email, etc.) are the user stories that enter the sprint backlog.

14. Customer Talks and Tests

Customer talks and tests are direct engagement activities with end users and customers designed to gather requirements, validate assumptions, and test product increments against actual customer needs. In PMBOK 8, this tool is recognized in the Stakeholder and Delivery Performance Domains as a practice for maintaining the customer-centricity that is foundational to value delivery in adaptive approaches.

“Customer talks” encompasses structured and informal direct conversations with current or prospective users: user interviews, customer advisory panels, product demos with active feedback collection, shadowing sessions (observing users performing actual tasks with the product), and co-creation workshops. These conversations surface the difference between what stakeholders say they want (stated requirements) and what they actually need when they interact with the product (latent requirements). This gap is the most common source of delivered-but-worthless features in software projects.

Customer tests

“Customer tests” refers to structured usability testing, acceptance testing, and beta testing with actual customers rather than internal QA. Customer tests are distinguished from internal testing by who performs them: real users with no product knowledge, using the product for real purposes, in real environments. Internal QA tests the product against specifications. Customer tests reveal whether the specifications, even if correctly implemented, actually satisfy the user’s needs and fit their mental model of how the product should work.

Common customer test formats include:

  • Usability tests: Observing a user completing specific tasks with the product, without coaching or assistance. The observer notes where users struggle, hesitate, make errors, or express confusion. These observations directly inform UX improvements that no internal team member could identify because they are too close to the product.
  • User acceptance testing (UAT): Customers validate that the delivered product meets their acceptance criteria before the product is approved for production deployment. UAT is the customer’s formal verification step.
  • Beta testing: A limited release to a subset of real customers before full launch, with structured mechanisms for collecting feedback on defects, usability issues, and missing functionality.

Frequency and integration with the sprint cycle

PMBOK 8 encourages integrating customer talks and tests into the regular sprint cadence rather than treating them as separate, periodic activities. Lightweight customer talks (a 30-minute user interview, a quick demo with three customers) can be conducted within a sprint without disrupting the delivery rhythm. Customer insights from these sessions feed directly into backlog refinement, ensuring that the next sprint’s stories reflect current user reality rather than assumptions that may be months old.

15. Continuous Improvement

Continuous improvement is the ongoing, systematic effort to incrementally improve project processes, practices, team performance, and product quality throughout the project lifecycle. In PMBOK 8, continuous improvement is not a technique with a specific set of steps — it is a principle that permeates adaptive and hybrid approaches, supported by the collection of tools described in this guide: retrospectives, daily standups, velocity measurement, and customer feedback loops all serve continuous improvement.

The concept has deep roots in Lean and quality management philosophy. The Japanese term “kaizen” (change for better) describes the practice of making many small improvements continuously rather than occasional large transformations. PMBOK 8 integrates this thinking into the Uncertainty and Team Performance Domains, recognizing that complex projects are too dynamic for static processes — teams must continuously adapt their methods based on what they observe and learn.

The Plan-Do-Check-Act (PDCA) cycle

PMBOK 8 recognizes the PDCA cycle (also called the Deming cycle) as the foundational framework for continuous improvement:

  • Plan: Identify an improvement opportunity and design a change. This occurs in retrospectives, after-action reviews, or in-progress postmortems when the team identifies a process problem and proposes a specific improvement action.
  • Do: Implement the change on a small scale (one sprint, one team, one process area). Limit scope to enable learning without large-scale disruption.
  • Check: Observe the results of the change. Did the process improve? Were there unintended consequences? The check step requires that the team defined measurable success criteria before implementing the change — otherwise “check” is subjective and uninformative.
  • Act: If the change worked, standardize it (update the team working agreement, process documentation, or Definition of Done). If it did not work, analyze why and either refine the approach or abandon it and try something different.

Continuous improvement in the project context

PMBOK 8 distinguishes three levels at which continuous improvement operates in a project:

  • Product continuous improvement: Incremental delivery of product increments, with each increment incorporating feedback from the previous one. The product improves sprint by sprint based on customer and stakeholder feedback.
  • Process continuous improvement: The team’s delivery process — how stories move from backlog to done, how testing is integrated, how deployments are executed — improves sprint by sprint through retrospective actions.
  • Team continuous improvement: The team’s skills, collaboration patterns, communication effectiveness, and cross-functional capability improve over time through deliberate learning, knowledge sharing, and performance feedback.

Measuring improvement

Continuous improvement without measurement is wish fulfillment. PMBOK 8 supports tracking process health metrics that reveal whether improvement efforts are working: velocity stability (is the team’s delivery becoming more predictable?), cycle time (how long does it take a story to move from backlog to done?), defect escape rate (what percentage of defects reach the customer?), team happiness index (a simple 1–5 self-assessment of team morale and energy), and sprint goal achievement rate (what percentage of sprint goals are met?). These metrics make the effects of continuous improvement visible and create the data-driven feedback loop that sustains the practice over the long term.

Continuous improvement and the role of leadership

Continuous improvement requires psychological safety, dedicated time, and organizational support. PMBOK 8’s Team Performance Domain explicitly places responsibility on the project manager and project sponsors to create the conditions in which continuous improvement can thrive: protecting retrospective time from schedule pressure, acting on team feedback quickly, removing organizational impediments that the team cannot remove themselves, and modeling the intellectual humility required to acknowledge and learn from failures. Without leadership commitment to these conditions, continuous improvement degenerates into performative retrospectives that produce no lasting change.
